Unity 5.2.1p1 and iOS 9.0.1 AdHoc install shows Greyed out Icon

Greetings,

We're Having issues with 5.2.1p1 when built for iOS AdHoc distribution and synced / installed through iTunes. Icon is greyed out and will not launch.

Any ideas?
